Immunizations are required for students to register and attend school in Arizona (A.R.S 15-871 & 15-874).
Additional Notes for Immunizations
Exemption forms are available from the nurse and must be filled out in person by parents for personal exemption (grades K-12), religious exemption (PK) or health care provider for a medical exemption.
Remember, these are with understanding of potential exclusions from school in the event of any outbreak of disease as determined by the Maricopa County Health Department.
